Geriatric Assessment Services - Providing Specialty Care for Older Adults

Medical and service providers are faced with the challenges and complexities of caring for older adults.  Excela Health’s comprehensive, multidisciplinary Geriatric Assessment Service has been created to assist the patient, his family and primary care physician by coordinating care and promoting a higher level of functioning for the patient.  There is also the opportunity to assess and address complex social issues, which can lead to better utilization of community resources and family education.     

What Are Geriatric Assessment Services?

Designed to assist physicians in the diagnosis, treatment and management of complex geriatric patients, this service provides:

• An evaluation of the patient’s medical, psychiatric and social needs
• An assessment of the patient’s need for services
• A coordinated care plan of recommendations, developed for the primary care physician’s review and implementation
